Our CARES program partners with New Hampshire's  Medicaid managed care organizations  to provide short-term

intensive home-based interventions to proactively reduce crisis situations, prevent emergency department visits

and shorten inpatient hospitalization stays.

Our experienced team of behavioral health clinicians will engage youth and families in the treatment process in 

order to create a plan that is individualized, holistic and trauma-informed. Youth and families will receive daily

wraparound support services provided by a multi-disciplinary team. Our clinicians utilize a strength-based

approach that is grounded in evidence based practices.

Admission Criteria

Youth ages 5-18 with mental health diagnoses, intellectual and developmental disabilities, traumatic brain injury, substance abuse disorder or post-traumatic stress disorder. Individuals must:

​

  •  Be a member of NH WellSense Plan, NH Healthy Families or Amerihealth Caritas NH.

  •  Show deterioration in functioning from baseline.

  •  Be at risk for placement in an acute inpatient setting or residential treatment facility.

  •  Be able to safely return home with intervention and support.

  •  Family is agreeable to engagement and supports for up to 14 days.

  •  Exhibit no suicidal or homicidal ideation at the time of the referral.

A clinician will contact you to schedule an intake with 24 hours upon receipt of referral.
