We are thrilled to announce that our Massachusetts DDS licensing visit went exceptionally well. Our team demonstrated their commitment to quality in their recent audit with one of our largest funding sources. We received a positive score in both licensing and certification. The licensing team was very complimentary about the internal communication within our teams! Pictured are our teams from the West, Northcentral, and South East- all of whom deserve a huge THANK YOU!

LifeConnections has partnered with the State of New Hampshire's Department of Children, Youth and Families Division to develop a service to support youth ages 17+ with intellectual and developmental disabilities and complex medical and behavioral health needs.

Our Specialized Family Foster Care Program is a community based family foster care option that provides a continuum of care for youth that are ready to transition from a staffed residential setting, out of state placement and/or in need of an individualized foster home with staffing and intensive wraparound clinical services. The specialized foster care model provides a youth with the opportunity to live in a private family home with experienced providers. Our specialized foster care providers may be families, couples or individuals that provide an opportunity youth to learn new skills, socialize, participate in community activities, develop natural supports and live a meaningful life in their community. This service is creating capacity within the system of care and providing a service option for youth who are ready for the next step towards self-sufficiency.

Our Communication Matters Program has recently expanded under the leadership of Christina Stockin, Manager and Founder of our Communication Matters Program! Her perseverance and steadfast advocacy in New Hampshire have opened doors to a program that wouldn't otherwise exist within the State. Our unique program located in Hooksett, New Hampshire will allow us to serve additional individuals who are Deaf with Intellectual and Developmental Disabilities. Our amazing support team consists of experienced ASL trained staff who are trained to provide culturally competent treatment and supports to help improve an individual's quality of life. We promote independence and empower individuals to live their best lives. Congratulations to our Communication Matters team!
